FOCUSED ON THE INDIVIDUAL
Intravenous
antibiotics are the most frequently ordered home infusion
therapy and can be the most complex. Crescent’s
experienced pharmacists and nurses promote patient safety
and positive outcomes by:
- Monitoring drug levels and lab data
- Minimizing drug side effects through proper drug selection,
considering patient specific culture and sensitivity
data
- Providing in-hospital patient training when possible
- Assuring patients have proper IV access before discharge
- Utilizing electronic automated pumps and multi-day
bags and cassettes
