Crescent Healthcare’s Home Parenteral Nutrition Program is designed to fit the individualized needs of our patients. Beginning at the point of referral, Crescent’s Home Parenteral Nutrition Team assesses the patient’s demographics to determine an individualized prescription based upon the patient’s nutritional requirements.

Patients who are receiving Home Parenteral Nutrition require skilled nursing, pharmacy and nutrition analysis. Assessment by our Board Certified Nutrition Support Dietitians is provided as well as personal patient contact to assist with improving therapy outcome and quality of life.

Crescent Healthcare’s goal is to provide safe, appropriate and high quality services to each of our Home Parenteral Nutrition patients.

Our Nutrition Support Pharmacists and Dietitians review Home Parenteral Nutrition patient referrals and are available for case consultation. Each patient receives on-going evaluation and monitoring. We provide comprehensive patient education, supply management and routine communication with follow-up to the patient’s physician. Our clinical expertise is backed-up by comprehensive policies and procedures. We have the capability to offer patient-specific compounding and in-home initiation of Parenteral Therapy.

TARGETED INTAKE
Our Intake Specialists are knowledgeable about parenteral solutions and supplies. They are also experts at insurance verification and government eligibility criteria.

The intake process for Parenteral Nutrition Therapy focuses on critical information to provide thorough clinical patient management. We use a dedicated formulary to ensure the best products are always available. We offer high quality home delivery with 24-hour clinical support, 7-days per week.

We are able to tailor a Home Parenteral Nutrition prescription for both Adults and Pediatric patients that assures safe and effective nutrition therapy in the comfort of the patient’s home.

ACCESS TO RESOURCES
Crescent Healthcare works closely with nutritional product manufacturers to make additional resources available, such as educational videotapes and literature for patients and healthcare professionals. We provide information about the Oley foundation, a support group for patients who receive parenteral and enteral nutrition and the American Society for Parenteral and Enteral Nutrition (ASPEN), a professional society dedicated to optimum nutrition.
